چکیده

Uremic neuropathy is the most common neurologic complication of uremia, and it affects more than 60% of patients with end-stage renal disease (ESRD) who undergo dialysis.(1) This type of neuropathy symmetrically involves the distal extremities, especially the lower limbs. On electrophysiological assessment, an axonal pattern is dominant. The clinical course and the intensity of uremic neuropathy vary from one individual to another. Coexisting chronic conditions such as diabetes mellitus can aggravate the symptoms,(2) while dialysis can stop or delay progression of chronic uremic neuropathy.(3) Complete improvement occurs after kidney transplantation.(4) Recently, acute and subacute neuropathies in ESRD patients have also been reported.(5-8) These conditions have a demyelinating feature and develop after the initiation of continuous ambulatory peritoneal dialysis (CAPD). The pathogenesis of these nonchronic forms of neuropathy remains unclear. We report 2 cases of subacute uremic polyneuropathy that improved following kidney transplantation.

Subacute polyneuropathy in a patient undergoing peritoneal dialysis: clinical features and new pathophysiologic insights.

Recently, demyelinating polyneuropathies have been reported in end-stage renal disease patients. These acute and subacute neuropathies share a demyelinating feature and may develop after the initiation of continuous ambulatory peritoneal dialysis. The pathogenesis of these non-chronic forms of neuropathy remains unclear. We report a case of subacute polyneuropathy that posed a clinical dilemma.

Outcome of Patients Initiating Chronic Peritoneal Dialysis During the First Year of Life.

BACKGROUND AND OBJECTIVE Among children with end-stage renal disease (ESRD), those who abstract initiated chronic dialysis during the first year of life historically were less likely to survive or receive a kidney transplant compared with those who initiated dialysis later in childhood. We hypothesized that recently treated infants have experienced improved outcomes. METHODS We queried the Nor...

Peritoneal dialysis immediately after kidney transplantation.

Approximately 10% - 20% of adult kidney transplant recipients and as many as 40% of pediatric recipients receive peritoneal dialysis (PD) before kidney transplantation. An important aspect of perioperative kidney transplant care is management of the PD catheter. Peritoneal dialysis can be performed immediately after transplantation for delayed graft function (DGF), which can occur with as many ...

Comparison of hemodialysis and peritoneal dialysis--a cost-utility analysis.

OBJECTIVE Our aim was to compare both health-related quality of life and costs for hemodialysis (HD) and peritoneal dialysis (PD) in a defined population. DESIGN Decision-tree modeling to estimate total costs and effects for two treatment strategies, HD and PD, among patients with chronic kidney failure, for 5 years following the start of treatment. Courses of events and health-care consumpti...

Sleep-disordered breathing changes after kidney transplantation: a polysomnographic study.

BACKGROUND Sleep disorders are common in patients with end-stage renal disease (ESRD) and are not improved by either conventional haemodialysis or peritoneal dialysis. Sleep-disordered breathing (SDB) is associated with cardiovascular disease and contributes to high mortality found in patients with ESRD. Cure of SDB after transplantation has been anecdotally reported. METHODS Thirty-four non-...
